2011-12 Autographed Ball Order Form 
CHAPEL HILL - The Carolina men's basketball team is now selling autographed basketballs signed by the 2011-12 team to the public. Head coach Roy Williams and the team will donate proceeds from the sale of balls to local charities.
Those interested in purchasing a ball will need to download the form from the Autograph Policy page on TarHeelBlue.com (under the FAN CENTER tab) on, and not before, October 1st.
Please note: walk-in purchases will NOT be allowed. Fans will need to complete and mail the form with a check (price is $175 and credit cards will not be accepted). Payment for autographed balls MUST be accompanied by a completed order form.
Each household may purchase two autographed basketballs. Buyers will receive ball(s) or returned checks within 2-6 weeks. Balls are available only while supplies last.
